CHES Hans Burgers Award Application Form

This award is presented to a resident of Canada as a mark of recognition of outstanding achievement in the field of healthcare engineering. Candidates must be members of CHES and the achievements can be either a series of contributions or one outstanding contribution.

To be eligible for the award the candidate must:

  1. Be a CHES member in good-standing.
  2. Be a resident of Canada.
  3. Demonstrate an achievement that is either a series of contributions or one outstanding contribution.
  4. Exceptions to the above could be made if the candidate is deceased at the time of nomination. The candidate would need to have been a CHES member within 36 months prior to the deadline of April 30th.
