By completing this registration form you provide permission for CHES to collect your name, title, organization name, contact information, dietary restrictions and emergency contact information for use by CHES for the purpose of the annual conference. Your name and dietary restrictions are provided to caterers and venues for the purpose of menu development. CHES uses QR codes, that can be scanned by exhibitors, and a conference app for program details. To provide you access to the app we require use of your name and email address. Within the app you can use privacy settings to hide this information. Your information is not kept beyond the end date of the conference.

Photos and videos will be taken at this event for the promotional and archival purposes of CHES. Any attendee who wishes not to be photographed or recorded must advise the photographer onsite during the event.

Payment total includes 5% GST

Cancellation Policy: Cancellation of registration must be received in writing (registration@ches.org) 30 days in advance of the conference start date for registration fees refunded. A processing fee of $75 will be charged on all refunds. No refunds after 30 days prior to the conference.
